What are key benefits?
In-network
• Eye exam: Once every 12 months, $20 copay
• Frames: $150 allowance every 12 months ($85 at Costco/Walmart/Sam’s Club), $20 copay
• Standard corrective lenses: Single vision, bifocal, trifocal, lenticular – $20 copay
• Lens enhancements: Polycarbonate (children), UV coating, tints included; other enhancements at negotiated copays
• Contact lenses: Once per year, elective allowance $150, necessary lenses covered after eyewear copay
• Laser vision correction: 15% off regular price or 5% off promotional price at participating locations
Other important info
• No claims filing needed in-network – just pay copay and any over-allowance amount
• Exclusions: medical/surgical eye treatment, prescription/non-prescription medications, lost/stolen lenses, elective treatments outside schedule, and services while outside the U.S. (except emergencies)
