• Willamette Dental – Willamette Dental locations in the PNW only

• Basic procedures – Fillings, extractions (copays or coinsurance apply)
• Major procedures – Crowns, bridges (coinsurance applies)
• Orthodontia – Available depending on plan (check plan details)
